My anger...oh man. I've got this aggitated depression/anxiety, and man it seems so hard to control. I take Seroquel, but it seems I have to take it 3-4 times a day to work..and it doesn't do *that* great of a job either. So, I took one of my Dad's perphenazine(sp?). It's an older typical antipsychotic, but it helps bring the temper and anger down. I can't take Zyprexa, which worked wonders for me, but it causes high blood sugar and I don't want to get diabetes.(I use my Dad's glucometer, and my readings when on Zyprexa are sky-high!) Here in Canada we don't have many atypical antipsychotics. Should I use the typical antipsychotics daily? I feel *so* bad when I stew in my anger...which IS daily..and this has been with me most of my life. Anybody else with this condition find relief? Thanks so much...

Trileptal.
Tegretol (carbamazepine) has an excellent history for treating anger and impulse control. Today, my guess is that Trileptal (oxcarbazepine) has replaced Tegretol as the first choice due to its milder side effect profile. With Tegretol, there is a very small risk for the development of agranulocytosis, which is a condition of abnormal white blood cell development. Tegretol requires biweekly blood-tests to screen for this condition. The only concern with Trileptal is that some people develop a reduction of serum sodium. Increasing salt intake and drinking less water should be sufficient as a prophylactic to prevent this from happening. It is probably a good idea to check from time to time with a blood test.
Some people with borderline personality disorder do very well on a combination of Trileptal and Zyprexa.

Scott -
I'm still fairly new to these boards, but have been browsing alot without posting much. I've particularly enjoyed reading many of your posts in response to others. You seem extremely knowledgeable, and I'm curious if your knowledge comes from personal experience or if your career is in the field of mental health.
It's also obvious to me that others on these boards appreciate your insight, knowledge and support.
Hope you don't think I'm flirting, LOL (happily married for many, many moons).

>Thanks very much for the post. I've tried going that route...with Ritalin and a brief time on Adderall (before they pulled it from the shelves here in Canada.) Both stims drove me, well, *nuts*! My anger turned to dysphoria, and ohh it was so bad. But, it was an angle I knew I had to try. Thanks for the suggestion...
Best,
Jay

> - ScottHey, thanks Scott. I was on Tegretol before, and I seemed to react to it pretty badly. But, that may have been because it didn't mix properly with other meds I was taking. I understand that Trileptal is less likely to react with other meds...and also I am on different meds then when on the Tegretol. More experiments...but fight the good fight, right? Thanks for your help...

Hi Jay :-)
How much perphenazine do you take?? Which of your symptoms does it help? Do you have any side effects?

Most of what I know I learned on my own or through the doctors I have seen through the years. Since severe depression began for me when I was 17, I didn't have much chance to develop a career, but I was leaning in the direction of becoming a lawyer, despite my passion for biology. After I learned that my condition was biological, I developed a passion for medicine and wanted to become a clinical psychopharmacologist. It won't happen. So, here I am playing doctor.

I know. What's that all about? Weird week.

I take 2mg's a day of perphenazine right now, and take 100mg's of Seroquel only at bedtime. The perphenazine seems to have fewer side effects than any other med, and I can tell it works. Let me know if there is anything else you want to know.
Best,
Jay :-)

Hey Scott, what does your significant other say about your flirting on the Boards. And BTW the pdoc who gave me trileptal said it was for sleep. So did he really think I was borlerline? None of the other pdocs do. He's the bad one being investigated. Fondly, Phillipa

Hi Jay,
2mg/day perphenazine is thought to be similar to about 0.5mg haloperidol (Haldol) per day.
>The perphenazine seems to have fewer side effects than any other med, and I can tell it works.
I think typical APs are often useful and well tolerated at very low doses, at least in the short term. In the long term, the risk of tardive dyskinesia appears to be very high.

Shhh! ;-)
> And BTW the pdoc who gave me trileptal said it was for sleep.
I haven't yet heard of using Trileptal specifically for insomnia. However, if one were manic or hypomanic, it is concievable that he was using it as a mood stabilizer, and expected insomnia to remit as the bipolar mania remitted.
> So did he really think I was borlerline?
It is difficult to guess what he was thinking.
We are fortunate that we are not permanently stuck with the same doctor.

I take geodon now in place of zyprexa and it seems to work for the most part. Not as good though. But it does work. I think its the sedative properties of zyprexa that are so effective. Geodon is not as sedative. Anyways you might try it. Its not bad for weight. And I doubt its bad for blood sugar. I do take a much higher dose of geodon and with less side-effects than zyprexa.
This is the end of the thread.
