Hi
I've been on nortriptyline for about 7 weeks now. My blood levels are in the therapeutic window - and it's helped my anxiety - but I'm still getting break through panic attacks. I'd like to increase the dose - but my PDoc seems reluctant to since the levels are in the therapeutic window. Is there any point in increasing dosage - or once you're in the window - do you not get any more benefit from the med?

Hi Jackie.The therapeutic for nortriptyline window is pretty wide. Blood levels can range from 50-150ng/mL. You may need to be at the higher end of the window to obtain optimal benefits. Most people end up taking 75mg. For me, 100mg was needed for me to receive any benefit. My blood level at this dosage was 123. Unfortunately, nortriptyline was only partially effective for me and not worth continuing. Perhaps you need another drug.
