You'll no doubt get some good first hand stuff here. But also, make sure to check out the textbook, Manic Depressive Illness by Goodwin and Jamison-Redfield. I usually read bits of it at Barnes and Noble. It's a big book, full of lots of great info on all aspects of the full spectrum of bipolar disorders. Well written, too.

Been there once for just over 24 hours. Do _not_ recommend it. I've read several other names for it: aggitated(sp?) depression, suicidal mania, among others. The concept as I understand is a predominant depression or mania infused with a trait from the other side of the mood spectrum. Anyway, my experience was mania with suicide ideation -- very scarey, very vivid thoughts. For me it was triggered by Prozac, so my treatment is to stay away from specific AD meds and work with meds that treat manic-depression. I have been told that rapid cyclers tend to get triggered into mixed states but don't know if this is true.

Good excuse to go check it out.Even Better--this book is available at public libraries for free and you can take it home, read at leisure, etc. I just had to remember that there are mild states of bi-polar and not to panic over the author's experiences necessarily being mine in the future. I would also like to know if the author's medicine ever pooped out! Anyone know? Or, did her hair fall out, did she gain lots of weight, etc.? Maybe I need to check it out and read it again because it has been a long time. BTW. Collette Dowling has an excellent book that helped me years ago. I'll have to go to the library and get refreshed!

Is it at the library? I tend to use B&N as a library lately. Been pretty delinquent in returning public library books, due to disorganization secondary to depression. I like the comfy chairs at B&N and the "being with people without being with people" and reading a small amount without feeling pressure to read more. Only problem...temptation to spend, money I don't have.

I went after work tonight. Sat in one of those comfy chairs. It's a big book. Not your run of the mill easy reading! I read some of it. However, it is set up such that looking for "mixed state" sent me all over the book (one place for assesment, one for symptoms, one for tx etc etc). I really either need to read more, or get a focused chunk of info on Mixed States from another source.

1. What is the difference between aggitated depression and mixed state bi-polar?
2. If a mood stabilizer is used, and seems to help, does that mean it is bi-polar? Or, can mood stabilizers work to make depression with aggitation better?
3. For those of you who are bi-polar--I know that antidepressants can send you into hypomania/mania, however I am curious about whether other substances that can act as stimulants can make hypomania worse? Can coffee, tea, chocolate in significant amounts increase the wound up feeling? And, has anyone noticed that hormones affect bi-polar status?
4. Any info on different mood stabilizers? ( (e.g. Efficacy, side effects)?

DO NOT MESS AROUND WITH A MIXED-STATE!!! Get in to your pdoc office ASAP. SusanJane is right on it when she says it's very scarey and may trigger suicide impulse. This was what happened to me when the overlying mania cycle into psychosis along with a ten-second rage. These few moments was all it took for me to overdose IMPULSIVELY on Seroquel.
Another thing. My mixed-states are almost always overwhelming with pain; angst, anguish, mental pain. Whatever! Call it what they will, but all I know is you can't miss it when you feel it.
To knock me out of a mixed-state, my doc has had me take atypical antipsychotics (Seroquel worked the best for me...Zeldox or Ziprasidone should be out soon. Zeldox was exceptional at pulling me out of the mixed-state and ultra-rapid cycling!)

I am also looking for information. After many years with a diagnosis of major depression (with some slight variations on the theme), I was just diagnosed as Bipolar with mixed states. I hope this isn't a fad diagnosis like Borderline Personality Disorder (BPD) was a few years ago. I was in a out-patient program in 1994 and everyone in the program had an Access 2 diagnosis of BPD! Anyway, for the mixed states,I was put on Neurontin as a mood stabilizer, gradually raising the dose from 900mg to 2000mg per day over the next two weeks. My Pdoc will most likely take it higher rhen. I can't tell you much about the side effects yet except that I am experiencing blurry vision and a slight sedation which are supposed to go away in a few days. I have found some possibly helpful information on the net that may help you.

The doc I see said there is a new book on bipolar disorder by Johns Hopkins. I saw it in the book store yesterday. It's very comprehensive but easy to understand. however, what you are describing and what I am being told about me (mixed state that is more depression with overlying aggitation) does not seem to be covered well anywhere. And there is not much on use of neurontin even though it is a new book. However I didn't read the whole book--just browsed and scanned. This seem to be a different "mixed state" from what the books and other posters are talking about.

I tried to post this once before and got an error message so I'll try again. Any help with questions below would be appreciated. After 3 years dx as major depression I have recently seen a new doc who dx me as mixed state bipolar!!! I need some feedback. The mixed states I see described here seem to be different from the overriding depression with constant inner aggitation that I feel. As in Annie's case the above is now being dx as mixed bipolar. Feedback?
Doc is using my reaction to AD's (instant overwhelming aggitation and sleeplessness from very short trials of Paxil, Effexor, Wellbutrin and to a lesser degree to Pamelor-none of which helped with the depression) and terrible postpartum depressions after each birth with the same sleeplessness and aggitation and depression)as a basis for this new dx. I have started taking neurontin but have had to quickly up the dose to maintain the effects--up to 2400mg now. Strange, usually I can only tolerate tiny doses of meds?? When it is working I definitely feel better, but after a few days at a new level it's effects start to decrease.

Just curious.No. He said since it was the only AD that had been helpful (helps with sleep) he would leave me on it and hope that the neurontin would settle things in me to a level where I could tolerate a higher dose of Remeron. If that doesn't alleviate the depression he would switch to something else. I seem to be able to soak up loads of neurontin without any real side effects. Each time I up the dose I get a glimpse of a pretty nice state, then the effects seem to start to disappear.

***************a definate "YES" caffeine and cocoa products will hype you up if you have bi-polar illness. If I want to get to sleep by 10pm I cut out coffee by 5pm. It's even better to drop off the caffiene in the afternoon. Depending on what pm drugs you are taking I'm sure this info may vary.

He put me on neurontin rather than depakote because of the side effect profile (depakote & weight gain). If I had to choose for myself which mood stabilizer to go with, it would be lithium. Unfortunately I can't use it alone at a maintenance level because of side effects.

How would you describe the difference in how lithium and neurontin affect you? I am now on 2400mg of neurontin. My system seems to just soak it up. ************I guess bi-polar and manic depressive are interchangeable diagnosis. If your one, then your the other. I find it all very confusing. I guess I actually have schizoaffective disorder, which means I take another type of med along with the "manic" meds.I just started taking neurontin and am only on the bare minimum dose of 400mg. He will probably up it next Dr.'s visit. I notice I am a little more aware now compared to when a just took lithium. I don't have that hazy feeling anymore.
Neurontin and lowering the lithium has knocked out my side effects (from lith.) completly. So far I'm happy with the situation. I guess time will tell if my progress continues.
Good luck to you and I hope you get a handle on your Doc's diagnosis. Don't be afraid to ask him pertinent questions that are bothering you. He/she may have "real" answers.

> LGHi LG. Theoretically Remeron is less sedating at high doses like 45mg or 60mg. I found it pretty sedating at 45mg but also very drugged during the day. I find it much more sedating at 7.5mg or 15mg without the daytime drugged feeling. But if it's helping with your depression, going down that low wouldn't be a good move.
This might sound strange, but it would probably work nicely. Ask your doc about one of the TCAs with a short half-life. All are sedating, but some more than others. I'm not sure which one that would be, but either you or the doc could research that stuff pretty easily on the web. The shortest half-life would avoid daytime sedation as much as possible.
I've never heard of a TCA being combined with Remeron. There's certainly nothing wrong with combining them. Especially if the TCA is added at a small dose just for sleep. It would potentiate the Remeron as well. And they're cheap. While the Remeron stimulates neurotransmitter synthesis, the TCA would provide some reuptake inhibition to boot. This probably is a rare combination because most people are already too sedated on Remeron to begin with. But if it's not doing that to do, adding the right TCA would make sense, even though for nearly all the rest of us Remeron alone is plenty sedating. I admit it sounds like a bizarre combination. But for you, and probably hardly anyone else, it could work well. You seem to have strong tolerance to antidepressant induced sedation. Just something to consider.

Hi LG,
I have been taking 30mg at bedtime for quite a while. Usually about an hour after I take it I can go to sleep OK. However, if I drink anything with caffeine or anything stimulating, I will still have trouble sleeping. I am now on 2400mg neurontin taken 4 times a day (2 capsules each time). I am not sedated durng the day. I am not sure if the Neurontin is doing what it is supposed to since I have never taken a mood stabilizer before. I think I am sleeping deeper and less racy and wired inside during the day.

sweets,but always have.Was told this might get

But I've been doing OK since doc changed some things for me.

I was searching the web for info on the question about neuroton and I was wondering if neuroton is prescribe for depression.
My cycles never seem to stop in the last year. I always was told I have bi-polar II but when my Dr fill a disability form it
put down depression as diagnosis. He did not back my disability claim and was really angry about having to fill out the form.
Also is anyone taking 600mg of welbutrin a day? I have had a real problem finding a Dr for my meds.
It seems like I am on an assembly line. Pay your co-pay ,see the Dr for 10 min. then out the door.I take 2400mg of neuronton,
600mg of welbutrin, 20mg of adderall and synthroid but I still have a lot of depression and suicidal thoughts or I am wired.
Every Dr I see has a different opinion about what is wrong with me. I feel like a lab rat. Thanks.
